Health Services: Student Immunization Requirements

Student Requirements

Any new student to the school system must present upon enrollment an immunization certificate. This applies to any new enrollee as well as Kindergarten and 6th grade students.  If the parent cannot present an immunization certificate, the student may not be enrolled, until an immunization certificate is made available to the school.

 

Any new student to the school system must also provide the school with a physical exam on the KY student physical exam form within 30 days from the date of enrollment.  This applies to Kindergarten, 6th grade and any new students.

 

Beginning in the 2021-2022 school year, any student involved in an extra-curricular activity must have an up-to-date KY immunization certificate on file at their school in order to participate.   Examples of extra - curricular activities include ( but not limited to) athletics, cheerleading, dance, band, color guard, FFA, clubs, choirs, drama, etc.
