Attendance Policy 2015

EJHS Truancy Procedures 2015-16

Parents are allowed to write three (3) excuse notes per semester for reasons they deem appropriate (illness, family emergency, college visit days, etc.).  Any other days beyond the three must be official absences (doctor, dentist, legal).  Each semester, absences beyond the three parent notes and official notes will be counted as unexcused.

All notes must be turned in to the office within 5 day of student’s return to school.  Any notes turned after the 5 day limit may be counted as unexcused.

Notes need to come to Mrs. Heard Office (rm. 801) - please make sure 1st & last name is on the note & place in basket. Some children have a different last name than the parent. It makes it very hard to figure out. So if you want to get credit for having a parent note or doctor note make sure your names are on it.  If they are not it will be put in, but it will show up as AUP-absent unexcused parent note or AUD- absent unexcused doctor note.  If you forget to send the note-stop by & tell me (if you are a student) call or email me & she will put a note in computer saying you forgot it & it is coming.

We can’t accept faxed notes. They can’t be faxed to your office & brought to me either. The doctor can mail to your house or the High school if you forgot it.

 You can only have 3 parent notes for each semester.

Unexcused absences will result in no credit for work done on the unexcused dates.

Unexcused absences may also result in the loss of driving privileges for students between the age of 16 and 18 under the No Pass, No Drive law.  For details on No Pass, No Drive contact Karyn Turner.  Parking Pass will be pulled depend on number of absences.

If a student signs out during the day & returns to school they MUST HAVE AN DOCTOR’S NOTE to sign back in.

Students CAN NOT SIGN OUT to go get lunch!

A note is required anytime a student misses part or all of the school day.

Other attendance issues will be governed by the policies of the Jessamine County Board of Education.

Medical and Dental Appointments
 A physician’s excuse shall state the date(s) and/or number of days for which the student will be excused.

a. Schools will no longer accept faxed medical statements.

b. Local physician's offices will use the standardized Jessamine County Schools' Medical Statement for Return to School form.

c. When a student exceeds 12 medical statements, the parent/guardian will be asked to sign a Release of Medical Information Permission Form for the treating physician/physicians. This form will allow the Health Attendance Committee to review health information for the purpose of:

*Determining the need for documentation regarding a Chronic Health Condition;

* Addressing the student's academic status;

* Determining the possible need for Home/Hospital services.

d. Each case will be reviewed on an individual basis.

Our highest priority for all students must be safety. We want parents to know the staff of EJHS will do our best to ensure students are supervised and accounted for at all times. With this in mind we must follow a clearly defined policy for students entering or leaving our building during the day.

Students sign out or in on the sheets in front office---MAKE sure we can read your name...if not it won't get entered in IC. You have got to put your last name on the sheet. Which means you may be counted absent all day or could be counted as skipping because we could not read the name to put in that you signed out.
