Effective July 1, 2018:
*All students, K-12, must show proof of having received two doses of Hepatitis A vaccine to attend school AND
*Students 16 years or older, must show proof of having received two doses of Meningococcal ACWY (meningitis) vaccine (MenACWY) to attend school. If the first dose of MenACWY was received at age 16 or older, the second does is not required for school entry
School based health clinic services provided by:
Due to Covid-19, School based Clinic visits are limited to sports physicals, school physicals, immunizations (after fall break), urinary symptoms, ear aches or wound care.
New locations and hours of operations! Students can get updated vaccinations; school physicals; sports physicals and be seen for certain sick visits as necessary. Contact Nurse Shepherd or call 887-9274 ext 4200.
Enrollment forms are filled out once per school year. Filling out an enrollment form does not mean you are obligated to be seen at a school based clinic; however, if you need to be seen quickly and can not get in with your primary provider, the school based clinic might be the answer!
